Alzheimer Disease is the most common chronic neurodegenerative disorder associated with aging. Nevertheless, its pharmacological therapy is still an unresolved issue. In double-blind controlled studies, acetyl-L-carnitine demonstrated beneficial effects on Alzheimer's disease. However, the mechanisms behind its neuroprotective ability remain to be fully established. In this study, the effect of acetyl-L-carnitine on amyloid precursor protein (APP) metabolism was investigated by in vitro models, both in a neuroblastoma cell line and in primary hippocampal cultures. We found that acetyl-L-carnitine treatment stimulates alpha-secretase activity and physiological APP metabolism. In particular, this brain product favors the delivery of ADAM10 (a disintegrin and metalloproteinase 10, the most accredited candidate for alpha-secretase) to the post-synaptic compartment, and consequently positively modulates its enzymatic activity towards APP. Our findings suggest that the benefits of acetyl-L-carnitine reported in previous clinical studies are underscored by the specific biological mechanism of this compound on APP metabolism. In fact, acetyl-L-carnitine can directly influence the primary event in Alzheimer's disease pathogenesis, i.e. the Amyloid beta cascade, promoting alpha-secretase activity and directly affecting the release of the non amyloidogenic metabolite.

Researchers evaluated the effect of acetyl-L-carnitine ( ALCAR ) vs. amisulpride measured by total Hamilton Depression Rating Scale score in patients with pure dysthymia. Two hundred and four patients were randomised and treated with ALCAR 500 mg twice a day or amisulpride 50 mg u.i.d. in a double-blind study, for 12 weeks. A solid improvement was observed in both treatment groups throughout the study. The results did not disclose statistically significant differences between treatments. The greater tolerability of ALCAR is of clinical relevance considering the chronicity of dysthymia, which often requires prolonged treatment.

ADD or ADHD treatment

Acetyl l-carnitine for infertility
Researchers at the
Polytechnic University of Marche, in Italy had an inkling that supplements could make a difference. They evaluated sixty infertile men between the ages of 20 and 40 who had poor sperm motility or movement. They gave these men the nutrients carnitine, acetyl l-carnitine, or a combination. The results showed that the combination of carnitine and acetyl l-carnitine had the best outcome with improvement noted in sperm motility and velocity. The antioxidant capacity of the sperm fluid also increased.
   It would seem reasonable for men who have undergone a medical evaluation for infertility and have not had success with the standard medical approach to try a combination of these two nutrients, particularly if analysis shows their sperm do not swim well or are slow movers. The ideal dosage is difficult to know, but a cautious approach would be to alternate taking 200 to 300 mg of acetyl l-carnitine one day and 200 to 300 mg of carnitine the following day. The dosage can be increased after a month or two if the results are not adequate. Higher doses could lead to overstimulation, restlessness, and insomnia.

Mitochondrial dysfunction and oxidative damage are highly involved in the pathogenesis of Parkinson's disease. Some mitochondrial antioxidants / nutrients that can improve mitochondrial function and/or attenuate oxidative damage have been implicated in PD therapy. However, few studies have evaluated the preventative effects of a combination of mitochondrial antioxidants/nutrients against Parkinson's disease, and even fewer have sought to optimize the doses of the combined agents. The present study examined the preventative effects of two mitochondrial antioxidant/nutrients, R-alpha-lipoic acid (LA) and acetyl-L-carnitine, in a chronic rotenone-induced cellular model of PD. We demonstrated that 4-week pretreatment with R-alpha-lipoic acid and/or acetyl-L-carnitine effectively protected SK-N-MC human neuroblastoma cells against rotenone-induced mitochondrial dysfunction, oxidative damage, and accumulation of alpha-synuclein and ubiquitin. Most notably, we found that when combined, R-alpha-lipoic acid and acetyl-L-carnitine worked at 100 to 1000 fold lower concentrations than they did individually. We also found that pretreatment with combined R-alpha-lipoic acid and acetyl-L-carnitine increased mitochondrial biogenesis and decreased production of reactive oxygen species through the upregulation of the peroxisome proliferator-activated receptor-gamma coactivator 1alpha as a possible underlying mechanism. This study provides important evidence that combining mitochondrial antioxidant / nutrients at optimal doses might be an effective and safe prevention strategy for PD.

We studied the effect of acetyl-l-carnitine plus nicotinamide on murine diabetes mellitus induced by multiple low doses of streptozotocin. Male C57BL/6J inbred mice were injected intraperitoneally with citrate buffer or streptozotocin (40 mg/kg) for 5 consecutive days, followed by injections of saline solution or acetyl-l-carnitine plus nicotinamide (50 + 25 mg/kg) from days 6 to 110. Four groups were studied: normal control mice (C), treated normal control mice (TC), diabetic mice (D), and treated diabetic mice (TD). The treatment with acetyl-l-carnitine plus nicotinamide induced remission of autoimmune type 1 diabetes in mice produced by multiple low doses of streptozotocin.
